一、Focus-PDCA模型
Focus-PDCA是在传统PDCA循环基础上扩展的质量改进模型，强调更系统的问题分析和流程优化。
1)F（Find）：发现问题
2)O（Organize）：成立改进小组
3)C（Clarify）：明确现行流程
4)U（Understand）：根本原因分析
5)S（Select）：选择改进方案
6)接着进入 PDCA 阶段进行计划、实施、检查和处理。
二、实践路径
1.发现问题（F - Find）
1)现状评估：针对全院上报不良事件上报的数据，与对照医院进行比较分析问题。常见的问题如存在上报数量少、类型不全、质量低、系统不便、流程复杂、管理分散等问题。
2)触发改进：意识到当前管理无法满足国家医疗质量安全目标（如目标七：提高不良事件报告率）。
2.成立改进小组（O - Organize）
可以由质控科牵头，成立跨部门、跨专业的持续质量改进CQI小组。跨部门小组一般可以包括医务科、护理部、药学部、输血科、设备科、院感科、信息中心、总务科等十多个部门代表，明确各自职责。
3.明确现行流程与规范（C - Clarify）
针对现有的不良事件发生情况进行现状调查，通过调查了解医护人员上报意识、知晓率、系统使用痛点等。
进而识别关键问题，从诸多问题中识别关键问题，如上报系统不便捷、流程复杂、缺乏激励、无统一管理、漏报率高。
4.根本原因分析（U - Understand）
通过数据分析，深挖上报率低的根本原因，可能的原因包括：
1)制度流程不完善；
2)缺乏便捷的信息化上报系统；
3)培训不足，员工知晓率低；
4)缺乏非惩罚性激励机制；
5)多部门协同管理机制缺失。
5.选择改进方案（S - Select）
针对根本原因分析结果，制定有针对性的改进策略，包括不限于：
1)建立三级管理体系，统一管理职责；
2)修订制度与流程，明确上报分类、路径和处理规范；
3)引入信息化上报系统（科进系统），支持网页+手机端多模板上报；
4)制定培训与考核计划，提升全员上报意识和能力。
6.计划与实施（P - Plan → D - Do）
进行PDCA循环，持续改进不良事件上报情况
1)P设定目标、修订制度、招标系统、制定培训计划、建立三级监管体系。
2)D引进上报软件，实现多端便捷上报；加强管理，建立预警机制，实行闭环管理；分层培训，持续对医护、行政、后勤等不同人群培训；修订制度，明确非惩罚激励、分类分级、反馈机制；落实三级检查，科室自查、职能科室督查、院级委员会评审。
7.检查效果（C - Check）
根据数据分析核查不良事件上报结果；验证上报类型全面性，质量提升，系统使用率等。
8.处理与标准化（A - Act）
形成制度化成果；构建统一上报平台；修订制度与三级管理体系；建立非惩罚激励机制；
定期召开季度/半年度会议分析数据、反馈改进。
