所有的医院都会成立医疗质量委员会，在院长书记的领导下该委员会对医院的医疗质量进行监测，一般按季度、年度、月度进行医疗质量情况汇总。
1.医疗质量相关指标
医疗质量相关的政策中经常会涉及一些医疗质量指标，其中有一些核心指标，每个医院都会进行监控。如效率指标（门诊人次、出院人次、平均住院日、床位使用率），安全指标（不良事件、院感感染率）、病案质量（甲级病案率、病案首页填写完整率）等。
2.核心制度执行情况
具体就是首诊负责制、三级查房、会诊、疑难病例讨论、急危重症抢救、查对、病历书写管理、值班交班等核心制度的执行情况的监控。
3.不良事件管理
不良事件上报是常规工作，预感相关指标监控与管理也是相当重要。
4.问题分析
针对上述各类医疗质量问题，需要进行根因分析。在常规管理中鱼骨图是一个有效工具，最终落实到具体的什么原因导致某项医疗质量指标不合规，进而为持续改进提供依据。
5.改进计划
每个医疗质量报告都需要给出一个下一期持续改进的计划，因为监控的结果可以由监控数据得到，导致问题的原因可以通过根因分析总结出来，那么下一步就是给出整改意见和具体举措。具体改进措施就是下一期需要采取的措施，需要落实到具体的时间节点。从而在下一期的质量汇报总对应查看该措施是否取得了一定成效，由此形成PDCA的闭环。
