1.问题背景
很多疾病都有不同的治疗方案，在基层全科医生一个人就给了治疗方案。但我们到了大医院，大医院大都是专科治疗，不同科室治疗方案不同，患者怎么选呢？医院管理者是否也进行过深入分析呢，不同科室不同治疗方案都会达到治愈的效果。这样对患者无疑是福音，那么同样治愈，不同科室不同方案是否存在费用上的差异呢？患者负担谁会更低？
2.模拟分享
为了探索上述问题，我们基于医院病案数据可以获取诊断信息、手术信息，手术等级等信息。
如果我们将神经外科、胸心外科、肝胆、脊柱、关节、肿瘤类的科室定义为高风险、高技术的专科外科、把妇产、肛肠等科室定位基础手术科室、将普外科（如创伤、骨伤、普外等）定义为综合手术科室，其他划分为其他科室。这样这些类别的手术等级分化很明细，如下图。
具体到疾病，疾病可以划分为常见病、复杂疾病、创伤类疾病、妇产疾病及其他。
常见病多发病包括：痔、阑尾、骨折内固定取出、清创、腱鞘炎、扁桃体、腺样体、子宫内膜、宫颈类疾病。
复杂高风险疾病，如肿瘤、癌、破裂、出血、坏死、感染、急性重症、休克等。
创伤类的疾病，包括骨折、损伤、创伤、脱位、撕裂等关键词。
妇产科相关疾病包括妊娠、剖宫产、胎膜、早产、子宫、卵巢、输卵管等关键词。
各类疾病的手术等级如下图：
上述分析对于医院运营管理来说，我们可以将基础科室那4%的四级手术、常见病多发病的5%的四级手术进行分析，开展病历核对等，是否会挖掘一些有价值的信息对于患者的教育、疾病的早期干预起到作用？
同时，对于复杂疾病，也有一些1,2级手术治疗。这里面是否有一些高价值的治疗方案？或者存在治疗不足？
此外，在医院也会经常遇到一个疾病可以多个科室进行治疗，如腰椎间盘突出问题可以看骨科、也可以看神经外科。呼吸暂停可以看呼吸科、也可以看耳鼻喉科等。
那么类似的问题，两个科室的治疗结果如何？那种方案更优？具体到医院，现在很多医院科室分组，那么会有更多的疾病在不同的医疗组得到治疗，医疗组之间的治疗结果如何？

以去除内固定装置为例，有很多医疗组或病区可以处理，不同的处理手术等级结果会存在少许差异。
治疗结果一样情况下，那么就可以进行患者负担方面的分析。
