大模型在医疗病历生成领域展现出巨大潜力，有望释放医生60%的文书工时，让他们能专注于临床诊疗。但就像硬币有两面，这项技术也伴随着诸多风险。

一、技术之殇：可靠性与安全性的隐忧

1.大模型生成病历存在内容幻觉问题，会生成与医学事实矛盾的虚假信息。幻觉问题源于训练数据的噪声和模型的过度泛化。

2.可解释性缺失也是一大难题。Transformer架构的注意力机制让医疗决策路径难以可视化，医生无法追溯诊断依据，难以验证结论合理性。

3.模型脆弱性同样不容忽视。

二、数据困境：隐私与质量的双重挑战

1.数据性风险主要体现在隐私泄露和质量缺陷两方面。隐私泄露有：

1）数据采集时未脱敏EMR直接训练；

2）模型推理中患者主诉包含身份证号等敏感信息；

3）输出内容里生成病历关联隐私信息。

上述行为违反《个人信息保护法》的知情同意原则。

2.数据质量也不容乐观。核心问题是目前数据标注还不健全，高质量的医疗数据集还有待建设，例如如将“Ⅱ型”误标为“Ⅰ型”，会导致很严重的数据质量问题。

三、临床危机

AI生成病历趋向模板化，生成的病例很容易千篇一律，同科室同诊断的病历之间重复内容过多，关键推理细节容易丢失。

长期来看，电子病历可能沦为“计费工具”，医生临床推理能力也存在退化风险。依赖AI生成初稿后，医生自主鉴别诊断能力下降，形成依赖AI的恶性循环。 

四、伦理困境

在伦理与合规方面，责任界定存在困境。模型开发者未标注置信度、使用医生未修正禁忌药错误、医院管理者未建立审核流程，分别在误诊诉讼、医疗事故仲裁、医保拒赔事件中暴露出责任不清的问题。

五、破局之路

面对重重风险，未来需要构建有效的风险控制框架。技术层加固方面，通过RAG医学知识增强等技术提升算法准确率，提高生成可靠性。

同时，数据治理闭环也至关重要。原始数据不出院，保障数据隐私与安全。临床审核流程需加强，增加AI输出鉴别诊断排除依据，实现推理留痕等内容，提升诊疗质量。

伦理合规体系建设同样不可或缺。建立《医疗AI证据溯源标准》，对错误病历责任分级，形成医院、厂商、医生共担机制。

一言蔽之，大模型在医疗病历生成中的应用，虽布满荆棘，但前景光明。
