一、基础信息填写问题

1. 年龄填写错误

• 年龄与出生日期、入院时间计算不一致。

• 例如：患者实际年龄为18岁，但病案中填写为0岁。

3. 住院天数计算错误

• 实际住院天数与入院、出院时间计算不符。

• 例如：实际住院5天，但填写为0天。

5. 新生儿体重填写错误

• 体重不在合理范围内或未按规范填写。

7. 性别、离院方式等字段缺失或填写不规范

• 必填字段为空或填写内容不符合标准。

二、诊断信息问题

1. 诊断重复

• 同一病案中多次出现完全相同诊断编码。

3. 诊断冲突

• 同时上传相互排斥或内涵重复的诊断。

• 例如：“慢性萎缩性胃炎”与“胃炎”同时出现。

5. 笼统诊断

• 诊断部位或临床表现不明确，影响分组准确性。

• 例如：“静脉血栓形成”未指明具体部位。

7. 联合诊断缺失

• 应使用合并编码而未使用。

• 例如：同时有“高血压性心脏病”和“心力衰竭”，应合并编码。

9. 诊断与患者基本信息不符

3. 手术冲突

• 手术部位或操作方式不明确。
