病案首页数据质量对医院十分重要。首先，首页数据是衡量医院医疗质量和管理水平的核心指标；与医疗付费与医保管理息息相关，DRGs、DIP 等医保支付方式完全依赖首页信息作为结算与分值依据；其次，医院等级评审、电子病历评级、互联互通成熟度测评等统计指标与首页数据强关联，三级公立医院绩效考核的指标也以首页数据为主要来源；同时，卫统报表、HQMS、抗肿瘤监测数据上报等国家监测上报数据也来自病案首页；另外，医院医院运营管理涉及的日常运营监测、资源配置、成本核算、绩效分配等内容也是以首页数据为基础。
病案首页数据质量的提升是每家医院的重要工作。针对病案首页数据质量问题，其提升的管理流程可以分为四大阶段、八个关键节点：
1.标准制定
依据国家卫健、医保、医院三方规范，统一疾病诊断、手术操作、药品耗材、科室人员等字典标准。一般可以由质控部牵头建立《病案首页数据质量指标》——含诊断、手术、基本信息、住院过程、费用等六大类校验规则。
2.数据预填
病案数据来自HIS、EMR系统；可以由门诊医生开电子入院证，提前录入患者基本信息。住院处也可以补充其余入院信息，自动同步到 EMR 病案首页，减少医生重复录入。
3.智能校验
医生在 EMR 中补录诊断、手术等医疗信息后，可以进一步调用病案首页数据质量校验规则，如病案质控产品等
4.科室初审
医生提交首页后，由科室病案质控员初审，审核不通过直接退回医生端并附修改意见；通过后流转至病案科。
5.病案科复审
病案科进行二次人工+系统复合审核；复审通过可以一键上传至国家/省级平台；不通过再次退回科室。
6.反馈通报
信息科联合病案科每季度生成《首页质量简报》，宣贯病案数据质量问题。将缺陷分类反馈给科室，纳入绩效考核。
7.绩效挂钩
将病案首页上传及时、质量达标的科室给予绩效奖励；连续排名末位的科室启动 PDCA 专项改进。
8.持续改进
召开专题会，分享改进经验；根据政策变化、临床需求更新校验规则，进入下一轮 PDCA 循环。
通过以上八个节点，病案首页数据实现了“标准制定-自动预填-实时校验-两级审核-统一上传-绩效反馈-持续优化”的完整闭环。
