一、 总体质量概况
2025年度，全院护理质量总体控制良好。在纳入监测的6项核心指标中，5项指标（83%）达成年度控制目标（绿卡），仅1项指标未达标（红卡）。

2. 指标达标情况分析：
从上图可见，除“跌倒/坠床”发生率（0.031‰）略微超出目标值（0.03‰）外，其余压力性损伤、用药错误及导管安全指标均处于安全范围内。
二、 具体指标深度分析
1. 跌倒/坠床发生情况（未达标）
全年累计发生5例。7月份出现峰值，导致当月发生率飙升，直接拉高了全年平均水平。
2. 用药错误发生情况
分析：全年累计发生2例。虽然前半年有零星发生，但第三季度（7-9月）保持了零发生记录。
3. 压力性损伤发生情况
全年累计发生5例，总体发生率0.06‰远低于目标0.3‰。相比2023年的高频波动，2025年的防控措施更为有效。
4. 管道安全管理（非计划性拔管）
（1）胃肠管：
8月份发生1例非计划性拔管，导致当月发生率出现峰值。但全年平均值仍达标。

分析：这两项高风险导管在2025年表现优异，截至9月均为零发生，维持在绿色安全区。
三、 总结与改进建议
